How We Work
General Care Philosophy
At The Resident Care Ltd, our staff are dedicated to creating a comfortable and safe environment where residents are encouraged to maintain as much independence as possible, while still receiving the support they need. This balance empowers residents to take control of their daily lives, while ensuring that assistance is always available when required.
Our approach to care promotes autonomy, dignity, and respect; while making sure each resident receives tailored support based on their individual needs.
Service Eligibility
To be eligible for services at The Resident Care Ltd, the following criteria must be met:
- Mental Health Diagnosis: Service users will have a mental health diagnosis and will be under the Care Programme Approach (CPA) with a recognized Community Mental Health Team (CMHT) and an assigned social worker.
- Social Care Needs: Service users will have identified social care and support needs.
- Initial Assessment: We provide an initial assessment to determine whether the service is a good fit for each individual, based on their unique requirements.
Referral Process
We follow a structured referral process under the Care Programme Approach (CPA) to ensure a smooth transition and comprehensive support for all new residents. Here’s how it works:
- Initial Assessment: The process begins with an initial assessment to determine the suitability of our services based on mental health needs, social care requirements, and the overall suitability for supported living.
- Keyworker Assignment: Upon referral, a keyworker is assigned to each individual. This person plays a vital role in supporting the resident, working alongside the Care Coordinator from the CMHT, and helping manage daily aspects of the care plan.
- Collaboration with CMHT: Our staff work closely with the Community Mental Health Team (CMHT) and the assigned Care Coordinator to ensure integrated, high-quality care.
Key Points of the Assessment
Our comprehensive assessment process is designed to be:
- Person-centred: The service user’s preferences and choices are at the heart of the care planning process.
- Comprehensive: We assess mental health, physical health, social care needs, and psychosocial factors.
- Collaborative: We involve the service user’s support network, as well as relevant professionals, to create a well-rounded care plan.
- Safeguarding and Best Practice: Our approach prioritizes safety and ensures that all care is provided in line with legal and professional standards.
